A reunion spanning 28 years! A child with hepatoblastoma visited a lifesaver pediatric surgeon in Shanghai

28 years ago, a pediatric surgeon saved a life and wrote a miracle of life; Twenty-eight years later, the "little patient" rescued in that year and his family braved a typhoon to Shanghai, and went to a grateful appointment. Recently, in the General Surgery Diagnosis and Treatment Center of Children's Hospital of Fudan University, Professor Xiao Xianmin and a former small patient revisited a story of two-way running between doctors and patients.

On the day that Typhoon Jojocao made landfall in Shanghai, a reunion after 28 years was taking place at the Department of Oncology, Children's Hospital of Fudan University. Xiao Hai (a pseudonym), an 8-month-old patient from Wuxi, came to Shanghai Children's Hospital for treatment because of hepatoblastoma, and the team of Xiao Xianmin, director of pediatric surgery at that time, carried out tumor resection for him. Now, that year's small patient has grown into a man of heaven and earth, because never forget the savior, in order to fulfill the wish, coincidence in the appointment of a reunion, so, there is this wind and rain in Shanghai to visit the scene of the chief surgeon expert.

 

On the same day, Wang Yi, president of Fudan Children's Hospital, Zhai Xiaowen, Geng Hongquan, Deputy Secretary of the Party Committee Dong Yinran, Li Kai, director of the Department of Tumor Surgery and a group of young doctors gathered together to witness this simple and warm "doctor-patient two-way appointment".

After many years of doctor-patient relationship, we meet again today. Seeing Xiao Hai for the first time, Professor Xiao Xianmin warmly shook hands with him, and immediately bent down with concern for his physical examination. The scar on his abdomen seemed to tell the story of the rescue of the hope of life ignited by the combined efforts of doctors and patients.

"How good, how strong!" Professor Xiao could not help but exclaim. Because Xiao Hai was weak when he was a child, his family cultivated his habit of exercise and actively participated in sports such as tennis. Working in software engineering, he is now 1.8 meters tall, sunny and strong.

A few days before the meeting, Professor Xiao Xianmin, who is 78 years old and has semi-white hair, found his long-cherished "surgical experience". Over the years, Professor Xiao has been accustomed to taking detailed notes on the diagnosis and treatment of some difficult cases, and has accumulated a thick stack. As a result of long-term storage and repeated reading, the paper has turned yellow, and the edge is somewhat warped. The operation about Xiao Hai, in his vigorous handwriting and detailed drawings slowly unfolded.

 

In September 1997, while taking a bath for Xiao Hai, his family accidentally touched a mass in his abdomen, so they went to a local hospital in Wuxi for treatment, and then transferred to a pediatric hospital in Shanghai for treatment. The results of the CT examination surprised everyone: Xiao Hai's right half of the liver was completely occupied by the tumor, and the liver blood vessels were squeezed to the side! After examination, Xiao Hai is suffering from hepatoblastoma, fortunately, lymph node metastasis has not occurred, there are surgical conditions.

Due to the imperfect medical conditions at that time, it was very difficult for an 8-month-old child to regularly remove the entire right half of the liver. Professor Xiao Xianmin, who was then the director of pediatric surgery, accumulated surgical experience of hepatoblastoma during his advanced study in Switzerland, so he volunteered to concentrate all the surgical forces to complete the tumor resection. On September 29, 1997, with the concerted efforts of doctors and nurses, Xiao Hai's operation was successfully completed without any risk, followed by chemotherapy and "eliminating the roots". For 28 years, there was no recurrence.

Recalling the disease that year, Xiao Hai's mother is still haunted: "If it was not found early enough, it may not be able to do the operation!" Xiao Hai reborn, until the third year of high school, only to learn that he had this experience, his mother told him, "Xiao Xianmin, a doctor in Shanghai Children's Hospital, is your savior." From then on, Xiao Hai's heart deeply remembered the name.

 

In July this year, Xiao Hai accidentally brushed the official account of the Children's Hospital of Fudan University on the social platform Xiaohongshu, so he left a message in the background with the attitude of trying it out, so he continued the doctor-patient relationship after 28 years.

That Professor Xiao Xianmin is oil painting "enthusiast", Xiao Hai specially prepared van Gogh's painting collection as a gift to meet, and Professor Xiao will also carefully draw a painting back to Xiao Hai as a memorial. When visiting the oncology ward, looking at a painting of Professor Xiao's copy of the old courtyard of the pediatric hospital hanging on the wall, the scenery of the past Fenglin Road, Xiao Hai's mother immediately remembered. "We stayed in one of the new wards in the surgery Building of the 97th Elderly Wing, which had just been built." Xiao Hai's mother pointed to the painting on the building said. Recalling the past, Professor Xiao could not help but exclaimed: "It was not easy for the small fur to grow into the tall man now! It's the same as when the children's hospital moved from the old campus to the new campus. What a change!"

In the ward, many small patients who are still fighting against tumors gather in the corridor with curiosity, looking at Xiao Hai's eyes or implied envy, or full of hope. See big brother sunshine handsome appearance, as if to see the disease, healthy growth of their own. Professor Xiao Xianmin and the director of tumor surgery Li Kai invited Xiao Hai to join the children's tumor "Sunshine Little Warriors" club as "big squad leader", hoping to bring inspiration and hope to more children troubled by malignant tumors through his inspirational story of cancer success.

 

According to reports, hepatoblastoma is the most common liver malignant tumor in children, accounting for about 90% of liver malignant tumors in children. In recent years, the incidence has been increasing. As a malignant embryonal tumor, it is more common in infants and preschool children, usually in children under 3 years old.

Professor Dong Yiran, deputy secretary of the Party Committee of Fudan Children's Hospital and the head of the department of tumor surgery, introduced that the early symptoms of hepatoblastoma are not obvious and it is easy to be ignored, but parents can get hints through the following signals: abnormal abdominal masses, children report abdominal pain, loss of appetite and weight, and jaundice. If there are abnormal signals above, it is necessary to go to the hospital in time to rule out the risk of tumor.

Dong Yiran said that with the development of medicine, the current treatment of hepatoblastoma has been relatively mature, and tumors can be detected by imaging examinations such as abdominal B ultrasound. There are also effective chemotherapy, targeted therapy drugs and mature surgical techniques, and liver transplantation can be considered for advanced patients.

Pediatric oncology is a national clinical key specialty construction project of Children's Hospital of Fudan University. Since 1987, the Department of Tumor Surgery of our hospital has set up a tumor clinic, and has rich experience in the diagnosis and treatment of hepatoblastoma. About 30 cases of hepatoblastoma are treated every year. The 5-year tumor-free survival rate is 85.2%, and the 5-year overall survival rate is 92.1%, which is equal to the international advanced level.
